Community Health & Hospice (CH&H) is a non-profit agency that provides home health care and hospice services to the greater Lakes Region of New Hampshire. Each year our staff of over 100 delivers approximately 50,000 home visits in the Lakes Region and travels nearly a quarter of a million miles annually in order to help individuals in need.
We do this by providing physical therapy, nursing care, pediatric care management, homemaker services, end of life care and much, much more. We visit individual’s right in your own neighborhood, right in their own homes, and we are available 24 hours a day, 7 days a week. Community Health & Hospice is part of the fabric of our community and has become so important in helping to maintain the dignity and independence we treasure here in New Hampshire.
Supported by the communities we serve through public, private and corporate donations, the agency is licensed by the state of New Hampshire and certified by Medicare for both the home care and hospice programs. CH&H maintains a voluntary accreditation through the Community Health Accreditation Program (CHAP), annually submits a Community Benefits Report to the State of New Hampshire and participates in regular Community Needs Assessments.

Center Harbor, Meredith, and Moultonborough Community Caregivers, a nonprofit volunteer organization, provides supportive services and information for our neighbors to assist them in their daily lives. Our program complements existing public and private resources and is geared toward helping neighbors remain independent in their own homes. There is no charge for services. Our support comes from charitable giving.
Community Caregivers was founded in 1998 as an outreach program of the Center Harbor Congregational Church, Center Harbor, New Hampshire. Of the first group of 25 volunteers, 6 served as coordinators. Working from their homes, these coordinators received requests from neighbors and contacted the volunteers who provided services to these neighbors. In 2004, Community Caregivers became a separate 501 (c)(3) nonprofit organization.
In the past year, our volunteers collectively drove more than 35,000 miles in their own vehicles and provided more than 1,100 supportive services to our neighbors in Center Harbor, Meredith, and Moultonborough. Community Caregivers is strongly committed to helping our neighbors to enjoy the caring support of their community.
